Short-Form Death Certificate vs Long-Form Death Certificate. A short-form death certificate is a comprised version of the long-form. It contains the most important information such as names, the date, and place where the death occurred, but does not include any detailed information, like cause of death, and reporting person. Each state has its version of short-form death certificate format, also often referred to as abstracts, because they only provide a fraction of information of the long-form. In some states, they are very similar to the long version and are even considered to be the standard size. A long-form death certificate often referred to as the official death certificate, and full-size death certificate, is a copy of the original document. It contains all the legal information, including signatures from the attending medical personnel. The long version also includes a file number, and the date when the death certificate was filed, the short form does not. Long form death certificates genuinely take much more time to process in terms of layout and formatting, therefore the translation costs a bit higher than one of a short form certificate.
